Physio – Self Referral

Patients can now self-refer to the MSK Physiotherapy Service.

Who is Eligible?

  • Anyone over 16 years old that is a patient of this GP practice who has pain or discomfort in their muscles, joints or ligaments.

Who is not Eligible?

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges.

Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates
  • Private letters

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
 

Healthy Routes Clinics

  • Weight management inc. healthy eating and physical activity (can tailor advice to pre-diabetics and those with raised cholesterol).
  • Stopping smoking and harm reduction (those wishing to cut down).
  • Alcohol reduction
  • Mental wellbeing

Health Promotion

We do not routinely provide Well Person Checks (well man / well woman) – however you can discuss any concerns with a GP or Practice Nurse during a routine appointment.

Influenza Vaccinations

Children

Children aged between 2-4 years are entitled to a nasal influenza spray, the practice will contact you to make an appointment with the Practice Nurse if you are eligible.

Adults

Adults who are entitled to receive an influenza vaccination are:

  • Aged 65 yrs and over
  • Patients aged 6m – 64 yrs who have a chronic disease
  • Pregnant ladies
  • Immunosuppressed patients and their household members
  • Patients living in a care home
  • Carers (receipt of carers allowance)
  • Patients with a BMI over 40
  • If you fall into any of the above categories please contact the practice from September onwards to make an appointment with the Practice nurse. If you are contacted and do not wish to receive the vaccination please let us know then we can update your records.

Chronic Disease Reviews

Patients are invited annually in their month of birth for reviews of the following long term conditions:

  • Asthma

  • Chronic Kidney Disease   
  • Dementia   
  • Diabetes   
  • Epilepsy     
  • Heart Failure

  • Rheumatoid Arthritis  
  • Stroke/TIA

You will receive a letter every 12 months around your birthday with an appointment to see the Nurse for your annual review. If you do not wish to attend please complete the exemption slip at the bottom of the letter and return to the surgery.

Please ensure that you have any bloods taken if needed before your appointment.

It is important to attend for these reviews so that your health and any medications can be reviewed to ensure you are receiving the correct care and treatment.

Cervical Cytology

Under the NHS Cervical Screening Programme, all women between the ages of 25 and 65 are regularly invited for a smear test The system is automated, so long as you are registered with a GP you should receive a letter asking you to make an appointment.

As a result of research that evaluated the optimal frequency for cervical screening, women are now invited for their first test at 25. They’re then invited every three years until the age of 49, and every five years from 50 to 64. From 65, only those who’ve had recent abnormal tests are offered another test. 

Cervical smear results usually take around 8 weeks, you will receive a letter at home.

We would like to stress the importance of women attending for smear tests when called to do so. They can pick up early changes and can help avoid cervical cancer.

Blood Tests

This service is now available 5 days per week at a number of community clinics in the Wigan & Leigh area including: Chandler House, Ashton Clinic, Pemberton Clinic & Standish Clinic.  The service operates on an appointment only basis, appointments can be made by online Appointments can be made online – https://www.wwl.nhs.uk/phlebotomy or by telephoning 0300 555 4567 – Option 3 (Mon-Fri 9am-5pm). 

Due to Covid-19 patients are unable to use the Walk-in service at the Thomas Linacre unless your bloods are marked as urgent by your GP.

Well Baby Clinics

Health Visitor Well Baby Clinics are currently running two clinics per week on an appointment basis only.

Wednesday 1pm – 3pm at Pemberton Clinic, Sherwood Drive

Thursday 1pm – 3pm at Chandler House, Poolstock Lane

Health Visitors can provide advice, support and information to parents to help them make decisions that improves the future health and wellbeing of their family.

6 Week Checks & Post-natal Check

Six week baby checks and mums post-natal checks are done by a GP, this is a combined appointment and usually takes 30 minutes.

The practice will contact you to arrange this appointment, please ensure you bring your babies “Personal child health record” red book with you.
